Common pitfalls

These are the patterns we see most often. Every one of them is avoidable with the right structure.
Automated test suites cover the happy path well. Edge cases, cross-module workflows, and user-specific data scenarios fall through — and surface in production.
Without a formally agreed test scope, testers raise issues outside the release perimeter. The cycle expands, the timeline blows out, and the go/no-go call gets harder.
You can count test cases run, but can you show what percentage of business-critical flows were actually validated? Coverage reporting that doesn't trace to business risk doesn't give you — or your stakeholders — the answer they need.
The SMEs nominated for UAT have day jobs. Participation drops off as the release cycle competes with BAU. Test coverage becomes a function of who turned up, not what mattered.
Production handoff happens on verbal sign-off or a meeting note. When a defect surfaces post-go-live, there's no documented record of what was tested, what was accepted, and what risk was explicitly acknowledged.
A long defect list arrives without severity grading. The QA lead has to triage 60 items under deadline pressure, often without the business context to make the right call on what blocks go-live.
